DOI 10.3109/17453670903039551 In the past few years hip spacers have become popular in the treatment of infected total hip arthroplasties (THAs), with reported infection eradication rates of > 90% (Anagnostakos et al. 2006). Two methods have been described for femoral fixation of a hip spacer. The spacer stem can be inserted into the femur either by a “press-fit” method, or by cementing onto the proximal femur (Anagnostakos et al. 2006). It is unclear which of these two methods provides superior fixation and thus acts better in the prevention of a spacer dislocation. In this report, we introduce a new technique for femoral fixation of an antibiotic-impregnated hip spacer.

Challenges of Plate Fixation for Vancouver Type-C Fractures after a Well-Fixed Hip Arthroplasty Femoral Stem

The fixation of distal femoral fractures (Vancouver type-C fractures) following a well-fixed hip arthroplasty femoral stem has become a challenging issue for orthopedic surgeons due to the inter-prosthetic biomechanical effects such as negative, positive, and torsional strain. Surgeons have applied a range of constructs to overcome these difficulties. To minimize the risk of inter-prosthetic fr...

Antibiotic impregnated total femur spacers: a technical tip

Simultaneous prosthetic joint infection of ipsilateral hip and knee arthroplasties is often accompanied by significant bone loss and presents a challenging reconstructive problem. Two-stage reconstruction is favored and requires the placement of a total femur spacer, which is not a commercially available device. We describe a surgical technique, reporting on 2 cases in which a customized total ...

Treatment of proximal femur infections with antibiotic-loaded cement spacers

In case of periprosthetic hip infections the implantation of antibiotic-loaded PMMA spacers is accepted for an adequate treatment option. Although their indication for the treatment of destructive, bacterial infections of the proximal femur would make sense, literature data are scarce. Hence, the aim of this study was to evaluate the efficacy of antibiotic-impregnated spacers in the treatment o...

Therapeutic Use of Antibiotic-loaded Bone Cement in the Treatment of Hip and Knee Joint Infections

The use of antibiotic-loaded cement spacers is an established method in the management of periprosthetic hip and knee joint infections. Despite inconsistencies among published studies, data shows that infection control rates exceed 90% with two-stage exchange arthroplasty. The present work reviews the current literature about antibiotic-loaded cement spacers and concentrates on the indications ...
